Anterior Lumbar Interbody Fusion (ALIF)
If you have a condition like degenerative disc disease (DDD) that affects the discs in your lower back, anterior lumbar interbody fusion may be the surgical option you need. This surgery provides optimal access to the L5 to S1 segment of the lower lumbar region, reaching the vertebrae from the front.
Dr. Rojeh Melikian, M.D., has specialized expertise in ALIF, and our team uses minimally invasive techniques to support better patient outcomes. Same-day surgery may be an option for qualified patients.
What Is ALIF Surgery?
ALIF is a procedure that most commonly accesses the L5 to S1 segment of the spine, involving minimally invasive techniques. The surgeon approaches the affected area from the front, through the abdomen, to avoid separating the back muscles. ALIF typically provides better stability for larger implants.
ALIF is often used at the L5 to S1 level in particular because the procedure allows access right below the split between major blood vessels. During surgery, an incision is made in the abdomen to access damaged or worn-out intervertebral discs. These discs are removed and replaced with bone grafts that will eventually fuse the vertebrae. The procedure aims to ease pressure on your spinal nerves, providing pain relief for your lower back.
Conditions Anterior Lumbar Interbody Fusion Treats
ALIF is most beneficial for candidates who have spine disorders that cause chronic spinal instability or lower back pain, such as:
- Degenerative Disc Disease (DDD)
- Lumbar radiculopathy
- Spondylolisthesis
- Failed back surgery syndrome
- Scoliosis
Dr. Melikian uses minimally invasive methods for L5 to S1 fusion to reduce pain, scarring and recovery time. We rigorously screen ALIF candidates to ensure outpatient surgery is safe for you.
Benefits and Risks of ALIF
ALIF can be a great option for many individuals with spine and back issues. It might be the right choice for your needs if you:
- Value minimal disruption to muscle tissue.
- Want a quick recovery timeline.
- Prefer same-day or outpatient procedures.
- Are cautious about traditional open-back surgeries.
- Do not want a large incision.
As with any procedure, ALIF comes with its share of risks such as blood loss and infection. In some cases, the procedure can also come with complications like nerve damage, incisional hernias, bowel disruption, and damage affecting the blood vessels or bladder.
Differences Between ALIF, PLIF and XLIF
Anterior lumbar interbody fusion, posterior lumbar interbody fusion (PLIF) and extreme lateral interbody fusion (XLIF) are all spinal surgeries that treat spinal disorders. However, there are a few key differences among each:
- ALIF accesses the lower spine from the front, whereas PLIF enters from the back and XLIF through the side of the body.
- ALIF typically involves less muscle disruption than PLIF, leading to fewer muscle injuries and a quicker recovery.
- XLIF surgery best accesses the upper lumbar levels, whereas ALIF is ideal for accessing the L5 to S1 region.
